    Facial Proportions

    The face encompasses proportions between its individual elements (forehead, eyebrows, eyes, nose, mouth and chin) which determine facial harmony and inherited beauty.

    All human beings possess the same elements and yet no two of them are exactly alike, including identical twins. To me this is one of the most fascinating mysteries of human beauty.

    The correct proportion between these elements is what determines whether a face is handsome or pretty. Often variances in millimeters between these elements make the difference between harmony or imbalance.

    The chin has an important function within this aesthetic and facial beauty.

    The positioning of the chin, a bit to the front or backwards is important to determine the correct face proportions. The chin should touch an imaginary vertical line that passes the top of the nose. Due to that, there is an important relation between chin and nose.

    Causes

    Deformities to the chin may be caused by:

    • Alteration during the bone growth period;
    • Defects acquired due to trauma to the face or tumors;

    Surgery Indication

    Surgery is indicated to correct chin deformities as follows:

    • Hypermentonism – advanced chin
    • Hypomentonism – weak chin
    • Lateral Deviation – chin that is deviated to one side

    The complexity of the surgery will depend on defect gravity.

    MENTOPLASTY – Advanced Chin

    The repositioning procedure can encompass only the chin or the whole jaw structure.

    Basically, an incision is made to the chin bone and it is placed backwards to the position which is defined prior to the surgery.

    The new chin is fixed by utilizing mini-screws.

    In this case, general anesthesia is used and the incision is made inside the mouth (near the lower lip).

    However, when the defect is more complicated and involves the jaw bone (mandible), the correction is made in the jaw angles region and in the chin region, as well.

    After that, the jaw bone is reshaped and repositioned and held

    together by mini-screws. Same procedure is used to hold the new chin.

    In this case, general anesthesia is used and the incision is made inside the mouth (near the lower lip).

    It is possible, through specific studies, to determine the new chin position

    to achieve the expected results and a more natural look.

    Post-Surgical Care

    1. Liquid and soft food diet is required for a few days after surgery due to the Intraoral scar.
    2. Swelling and bruising will disappear in approximately 20 days.
    3. It will be possible to return to normal schedule after 10 to 15 days.

    MENTOPLASTY – Weak Chin

    In this case the correct diagnosis is very important to understand if the patient has a weak chin from a small lower jaw or if it is an isolated weak / small chin.

    Weak chin correction is relatively easier when intervention is restricted to

    chin region.

    By moving the chin structure forward, it becomes possible to insert implants either made of solid silicone or porous high density polyethylene.

    In this case, general anesthesia is used and the incision is made inside the mouth (near the lower lip).

    It is also possible to move the chin forward by an incision to the bone, then the bone segment is dislocated and held together using plates and screws.

    In this case, general anesthesia is utilized and the incision is made inside the mouth (near the lower lip).

    However, when the defect is more complex and involves the jaw bone (mandible), then the correction procedure is made into the jaw angles bone region and in the chin region too.

    The jaw bone is repositioned and it is held together by using mini-screws, as well as the new chin bones.

    MENTOPLASTY – Deviated Chin

    The jaw bone is repositioned and it is held together by using mini-screws, as well as the new chin bones.

    Risks

    Infections: sometimes it becomes necessary to remove the implant (silicone or porous high-density polyethylene).

    Although this is rare, procedure for removing implant occurs after infection is healed.

    Relationship between the Nose and the Chin – Profileplasty

    1. Proportion between the nose and chin is closely related.
    2. The positioning of the chin, is it more forward or further back, has fundamental importance in determining the proportionality in the face.
    3. The chin should be touching a vertical line through the root of the nose.
    4. Similarly, the chin has a direct proportion to the nose. Sometimes the patient feels that his nose is large, and in fact the chin is too small. Thus, correcting the chin with an advance helps to improve the nasal balance.
